Torsion of a normal ovary occurs rarely, especially in girls younger than 15 years of age. In this age group, ovarian torsion is most frequently a complication of an underlying benign and occasionally malignant neoplasm. Management of these patients may be conservative considering future fertility and hormonal utility of the ovaries. Intraoperative pathological evaluation has great importance to protect the residual ovarian tissue. In this report, a seven years-old girl with a right ovarian torsion with multiple simple cysts and a left ovarian serous cystadenoma is presented and diagnostic peroperative challenges are mentioned.
